Georgia Health Imaging has been working with law firms across the country for many years. Our board certified radiologists provide detailed reports for all personal injury, workman's compensation, and other legal cases. 

 

In order to ensure patient's privacy is protected, please complete the record release form and to return it to our office. You are welcome to fax it ​to us at (678) 924-0965 or mail it to our office address.

If you need to obtain full medical records for your client, please complete the following information in addition to the records release form and either:

 

1) fax it back to our office if the documents DO NOT need to be certified or

2) mail it to our office if the documents DO need to be certified.
