Before Your Appointment

To save time when you check in, you can download, print, and complete your patient forms at home.
Please bring them with you to your appointment.
Specific preparations are needed for certain exams to ensure the highest quality images.

Please bring the following items with you to your appointment:
- Your Doctor’s order/referral for the exam.
- If you are using insurance, you will NOT be seen without an order.
- A valid photo ID (i.e. driver’s license, passport, etc.)
- Your current insurance card (physical or digital are accepted)
- Previous exams or reports if you need to compare results.

If you are a self-pay patient, as in you are not using insurance at all, you DO NOT need a doctor’s referral for an exam.
If you are going through insurance, Yes, a referral (also called an order) from a licensed healthcare provider is required for all diagnostic imaging exams. This ensures you receive the correct scan for your specific medical needs and allows us to send the results back to your doctor.

We accept a wide range of major insurance plans. To confirm if we are in-network with your specific plan, please call our office. Our team can quickly verify your coverage for you.
We do accept Medicaid, Medicare, Aetna, Blue Cross Blue Shield, Cigna, UHC, UMR and more!
We do not accept Oscar, Kaiser Permantante or Tricare.
We are committed to making healthcare accessible. We offer affordable, transparent self-pay rates for patients without insurance or those with high-deductible plans.
