We are happy to do the pre-certification for patients. To ensure they are done on a timely manner, include the following:

•  Completed referral form with referring physician's NPI.

•  All relevant diagnosis codes. 

•  Relevant clinical notes 

•  Patient insurance information

•  Patient contact information

All MRIs and CT scans include a complimentary CD with the patients images. If you need a CD for X-Ray or Ultrasound exams, please indicate on the referral form. 

​

STAT exams are available with a 2-hour turnaround.
